Tranilast, an antioxidant and a derivative of the amino acid tryptophan, is an anti-allergic agent that seems to relieve many conditions caused by inflammation. Users report that it eliminates discomfort within minutes if it’s applied immediately after injury or at the onset of symptoms. We have found that it has a wide margin of safety and negligible toxicity, so it seems that tranilast can be used as needed to treat a wide variety of conditions such as hives, bronchial asthma, nasal congestion, scars, keloids, and skin contractures. In topical form, it can also provide welcome relief from minor afflictions like the pain of superficial burns, the itching of aggressive hives, and the discomfort of seasonal nasal congestion. The following reports describe the use of tranilast to treat 3 common but uncomfortable afflictions.

Tranilast for Hives
One of my clients, a 64-year-old woman in good health, noticed itching and redness on one shin several hours after she had eaten fresh strawberries.  Hives developed overnight on that shin. Although she was treated with loratadine (Claritin) once daily for 5 days, the hives continued to spread in an area that increased in size each subsequent day and itched severely. The itching intensified at night, and soon even the gentle touch of clothing and bathing or showering with warm water had become painful. Five days after the initial outbreak of hives, the entire front of the afflicted leg was covered in lesions that produced itching of increasing intensity.  On the fifth day of the outbreak, topical tranilast 10% in a simple vanishing cream base was applied twice daily and was rubbed into the hive-covered area. Within 24 hours of the first application of tranilast, the hives had faded and the intense itching had begun to resolve. Topical tranilast was applied twice daily as described for 9 additional days, by which time the hives had completely disappeared. It’s possible that the symptoms described would have resolved within that period of time without the use of tranilast, but improvement occurred very shortly after the first application of the tranilast-containing cream, so we attribute the improvement to tranilast.  No adverse effect of treatment was noted, and the client described topical tranilast as “a blessed solution” to her skin problem.

 Tranilast for Nasal Congestion
An 18-year-old client prefers a topical treatment for her seasonal nasal congestion, which she usually experiences during the spring and autumn.  She applies a pea-size amount of topical tranilast 10% in a vanishing cream base to the outside of each nostril and around the sinuses to prevent a stuffy nose. She applies the tranilast once in morning and once at night for 2-week periods and as needed.  Her nasal congestion resolves a few minutes after the application of the topical tranilast, and relief persists for about 3 hours. She’s experienced no adverse treatment-related effects, and she continues to find topical tranilast extremely effective in preventing and relieving her nasal congestion.

Tranilast During Air Travel
After having heard that tranilast helps prevent and relieve ear and sinus problems during airline travel, I tried a topical form during a recent flight. I rubbed a small amount of the cream over my sinuses and behind my ears and was pleased to find that it really worked; no nasal congestion developed,
and my ears didn’t pop or get clogged as they usually do during a flight.

We began to look into why tranilast seems to be so effective and found that it inhibits the release of substances such as histamine and prostaglandins, which often cause the symptoms of allergy (inflammation, swelling, itching). Further investigation revealed that topical tranilast has demonstrated effectiveness in treating scars, keloids, and other skin disorders.  In one study, a 14-year-old boy with localized scleroderma (a chronic disorder that involves the hardening and thickening of the skin) experienced what the authors termed “a dramatic improvement in contractures” after treatment with tranilast.

I discovered another use for tranilast when I sustained a second-degree burn to the back of my hand after inadvertently touching the side of my furnace chimney during winter. I applied tranilast 10% cream to the burned area right away and noticed the immediate relief of pain and redness. Healing was evident a few days later, and 6 days after the burn, I had no sign of injury.

We have found that topical tranilast (as opposed to oral dosage forms) provides fairly good and prompt relief when applied as needed. We usually recommend applying a 10% topical tranilast cream in an amount that would fit on a fingertip and reapplying the cream as needed. We are not aware of any particular upper limit to the number of times that tranilast can applied in a day, but from our limited experience, it seems safe to use in almost any amount.
